Breast Clinic

Sancta Maria’s Breast Clinic is a one-stop service for patients who may have found a lump and need further investigation.

For patients with symptoms such as breast pain, lumps, breast skin or nipple changes, we offer comprehensive rapid breast services, performed either all on one day or within 3 days.

Our one stop Breast Clinic uses the triple assessment approach. This is recommended for the investigation of breast changes or symptoms and has been shown to be the most effective means to reach an accurate diagnosis rapidly.

Triple assessment consists of

1. Clinical examination of the breast

2. Radiology comprising of mammography and or Ultrasound

3. Biopsy, if required. Please note not everyone will need a biopsy. This will depend on your symptoms and the findings of the radiological imaging (mammogram/ultrasound).

Mammography, if required, will be performed prior to your attendance at Sancta Maria Hospital using digital mammography, which is superior to standard mammography equipment still in use in some hospitals. Digital mammography is a very low radiation dose x-ray technique involving compression of the breast tissue between two plates of the x-ray machine for a few seconds. A radiographer performs this test and will check the technical quality of the films. The Radiographers are highly skilled in performing mammograms and will ensure that every care is taken to keep discomfort at a minimum. The Consultant Radiologist, Dr Patel, with over 20 years of expertise in breast diagnosis will interpret your mammograms in readiness for your subsequent appointment.

At Sancta Maria at our breast care clinic, our consultant breast surgeon, Mr Muralikrishnan with extensive experience in breast cancer ( and other problems) diagnosis and treatment including oncoplastic surgical techniques. He will examine you to look for any abnormalities, including lumps or other changes. A female chaperone will be present throughout your examination.

Ultrasound will then be performed by Dr Patel and this compliments the mammogram and clinical examination. Ultrasound uses high frequency sound waves (no radiation involved) to examine the breast tissue. An ultrasound scan is painless and it can generally be done in a few minutes. Ultrasound may be the only imaging test performed in younger (under 40) or pregnant women. Ultrasound is most useful in providing information about lumps or localized areas of breast changes.

Both consultants are present to discuss the result of your examination and imaging investigations and further management as necessary.

In a minority of cases a biopsy is required to further investigate findings of the mammogram and ultrasound. This will be undertaken on the same day you attend Sancta Maria Hospital under ultrasound guidance. The samples will be sent away for analyses by the pathology department . If you have had a biopsy your case will be discussed at a multidisciplinary breast team meeting (Breast MDT). This team consists of healthcare professionals – doctors (surgeons, radiologists, pathologists and oncologists), nurses, technicians and support staff. All are specialists in their field and will discuss the findings of your biopsy and further management. Following this meeting you will be informed of the results and outcome of the discussions by our doctors who are also members of the breast MDT.

Your Consultant will explain and discuss your results and any follow-up care you may need. If your results confirm that you have a breast cancer your diagnosis and treatment plan will be discussed with you by your Consultant and you will be introduced to a Breast Care Nurse.
Please note that for the majority of women the result of your assessment will show a benign (harmless) breast condition.

An endoscopy is an investigation which is recommended to gain views inside the body. An endoscope is a long, thin flexible tube with a fibre optic light source and video camera attached. The images obtained during the endoscopy are viewed by the Consultant on a screen.

An endoscope may be inserted through a natural opening in the body such as the mouth or anus and endoscopic instrumentation is also used in keyhole surgical procedures such as a laparoscopic cholecystectomy (gall bladder removal) via a small surgical incision.

An endoscopy is routinely performed under a local anaesthetic or sedative medication.

An endoscopic procedure may take 15 to 60 minutes to perform and is undertaken as a day case out-patient procedure, not requiring an overnight stay in hospital.

An endoscopy is usually a safe procedure, but complications can include a risk of infection or bleeding in the area examined.

There are a number of endoscopic procedures available:

Arthroscope: Examination of a joint.

Bronchoscope: Examination of the lung and airways.

Colonoscope: Examination of the bowel (colon).

Cystoscope: Examination of the bladder.

Gastroscopy: Examination of the stomach.

Hysteroscope: Examination of the uterus (womb).

Laparoscope: Used in keyhole surgery, for example a cholecystectomy (gall bladder removal).

Sport and Exercise Medicine

Sport and Exercise Medicine (SEM) became a recognised medical speciality in 2005 as a need was identified to manage SEM- specific problems that in the past had been part dealt-with by GPs and other specialists.

SEM Consultants commonly assess sports injuries, diagnosing and managing these as necessary. The majority of sport and exercise injuries are non-surgical meaning a SEM Consultant is ideally placed to manage them and coordinate rehabilitation. Patients that subsequently need surgery are investigated appropriately and referred on when needed. Many conditions that are not directly related to sports injuries can also be appropriately managed by a SEM doctor. These conditions often are called musculoskeletal problems (MSK).

Dr Mark Ridgewell offers the opportunity for patients to have a specialist consultation with a doctor who has been recognised by the GMC as a Specialist in SEM. This means that they will have had extensive training and hold considerable experience across various aspects of SEM.

Other medical issues that Dr Ridgewell consults on include:

• Pre-event screening and medicals
• Exercise prescription
• Managing medical problems in sports such as asthma, diabetes, epilepsy
• Under performance and fatigue
• Female health issues in sports

Steroid Injections

These can be useful to reduce inflammation or swelling in a joint or soft tissue. They are often used for shoulder, elbow, wrist, hip, knee and ankle problems. The injections are general very safe and are mixed with a local anaesthetic.

Hyaluronic acid injections (HA)

HA injections have been developed over recent years and are a useful treatment option for some conditions. HA is normally present in the synovial fluid of joints in the body. In Osteoarthritis it has been found that there is lack of this fluid and this can cause pain often seen in arthritis.

HA injections aim to restore some lubrication of the joint and can give pain relief for many months especially in mild arthritis. They are useful when a patient has osteoarthritis of a joint to delay any possible need for surgery such as joint replacement. This may be appropriate in a young patient when it is best to delay joint replacement for as long as possible or when playing sports and wishing to continue through the season.

ESWT (Shockwave)

In recent years it has been found that shockwave can be very effective in the treatment of various sports injuries and musculoskeletal conditions. It uses a high energy short burst of sound wave and is delivered to the area affected. Most conditions need between 3 and 5 sessions on a weekly basis to maximise the effect. It is thought that the treatment works but re-starting the healing process and by affecting pain signals in the body. It is commonly used and is accepted by many specialists as a useful option in some conditions.

It is mainly used in:

• Tennis and Golfer`s elbow
• Shoulder tendon problems especially when there are calcium deposits
• Patellar tendon pain
• Achilles tendon pain
• Plantar Fascia pain
• Shin splints
• Hip tendon problems
